• Missing 'text' and 'image_url'/'image_base64' — returns error because neither encode nor decode target is provided
• Unsupported image format for decoding — only PNG and JPEG are accepted
• Invalid or unreachable image_url — returns error if the image cannot be fetched
• Malformed base64 string for image_base64 — returns decode error
• QR code unreadable in image (too blurry, low contrast, or partially obscured) — returns error or empty result
