Clive 2014 LENT score for malignant pleural effusion: pleural LDH IU/L (<1500 → 0, >1500 → 1), ECOG 0-4 (0→0, 1→1, 2→2, 3-4→3), serum NLR as a number (<9 → 0, >9 → 1), tumour type (mesothelioma/haematological 0; breast/gynaecological/renal_cell 1; lung/other 2). Score 0-7: 0-1 low, 2-4 moderate, 5-7 high. Equality at 1500 or 9 is not scored. Not a diagnosis or treatment order.
Output

Side effects
- Missing required parameters (pleural_ldh_u_l, ecog, nlr, tumor_type) returns a validation error
- ECOG value outside 0-4 range rejected
- Pleural LDH exactly equal to 1500 or NLR exactly equal to 9 is not scored and returns an error or indeterminate result
- Invalid tumour_type enum value returns a 400 error
- NLR passed as a percentage rather than a ratio will produce incorrect scoring
