PESI (Pulmonary Embolism Severity Index) from Aujesky 2005: age in years plus male sex (+10), cancer (+30), heart failure (+10), chronic lung disease (+10), heart rate >=110 (+20), SBP <100 mm Hg (+30), respiratory rate >=30 (+20), temperature <36 C (+20), altered mental status (+60), and SpO2 <90% (+20). Classes I (<=65) through V (>125). Only for already diagnosed PE. Not a diagnosis, not an outpatient protocol, not sPESI, and not the 17-variable laboratory model.
Output
Returns the total PESI integer score and corresponding risk class (Class I ≤65 through Class V >125), reflecting 30-day mortality risk for patients with confirmed pulmonary embolism.

Use this endpoint when you need to compute the classic Aujesky 2005 PESI score (11 variables, no labs) for a patient with an already-confirmed pulmonary embolism diagnosis. Prefer this over sPESI when full PESI granularity is required or when institutional protocols specify Aujesky 2005 criteria. Do not use for PE diagnosis, outpatient disposition protocols, or when the simplified 6-variable sPESI is required.
Side effects
- Missing required parameters (age, sex, vitals, comorbidities) returns a validation error
- Out-of-range values (e.g. age <18 or >120, SpO2 <50 or >100) are rejected
- Using this for PE diagnosis rather than post-diagnosis stratification is a clinical misuse — the API computes only; clinical judgment is required
- Confusing this with sPESI (simplified PESI) or the 17-variable laboratory model will yield different risk classes
- Payment failure (402) if x402 micropayment is not completed
