For 1 to 10 NPIs, the CMS NPPES status of each (active or deactivated, with deactivation and reactivation dates), provider name or organization, practice city, state and ZIP and taxonomies, plus any HHS-OIG LEIE exclusion listed under the NPI, as facts from the files: not a credential check and not a determination about the provider.
Output
A JSON object with a results array, one entry per NPI, each containing: NPI status (active, deactivated, or not_found), provider or organization name, entity type, practice city/state/ZIP, taxonomy codes with descriptions and license info, enumeration date, deactivation and reactivation dates, replacement NPI if applicable, and an exclusions array of any HHS-OIG LEIE records matched by NPI. Also includes data_as_of timestamps for both NPPES and LEIE files and a coverage disclaimer.
