Side effects
- File too large (inline base64 must be ≤4 MiB; upload key path supports ≤50 MB)
- Document format not supported by Textract
- Questions array empty or exceeds 15 items
- Question string too long (max 200 chars) or too short (min 1 char)
- Low-quality scan yields low-confidence or empty answers
- Network or upstream Textract service error
