Side effects
- Invalid NCT ID format (must match ^NCT\d{8}$) returns a validation error
- Study not found on ClinicalTrials.gov returns null fields with UNKNOWN comparison values
- Submitting more than 3 studies in one request returns a schema validation error
- ClinicalTrials.gov API unavailability may result in failed upstream call
- Null previous_status or previous_update_date causes UNKNOWN comparison outcomes rather than an error
